DEMENTIA CARE

Pathways Neighborhood for Dementia Care Featuring The I’m Still Here® Approach

The Cohen Home utilizes the I’m Still Here® approach for those living with Alzheimer’s and other forms of dementia. I’m Still Here® is a philosophy of dementia care based on the belief that every person living with the disease is still there and can experience a high quality, dignified life regardless of the severity of their memory loss.

With this exclusive program, we gain a deep understanding of each resident’s personal story, strengths and preferences to create a purposeful and uplifting experience each day. The I’m Still Here® approach at The Cohen Home blends proven best practices in dementia care with Jewish HomeLife’s unique and long-standing commitment to make our residents our number one priority.

Jewish HomeLife is the only senior care organization in Georgia (and 1 of only 34 in the country) to have certified I’m Still Here® Centers of Excellence in our family of services. Pathways Neighborhood for Alzheimer’s and Memory Loss.

Our unique Pathways neighborhood balances independence with our highest level of supervision for those with Alzheimer’s or other forms of dementia. These residents still have their own private apartments, yet benefit from the higher staff ratio and activities tailored to their specific needs. Some Pathways residents continue to enjoy meals in our main dining room, but for those who benefit from less distractions or require greater assistance with eating, our Pathways Great Room provides a perfectly intimate setting.

      Inclusion Model Inspires Socialization.

Located on our main floor for greater accessibility, the Pathways neighborhood is an extension of our community, providing a more intimate setting for the residents. Pathways residents continue participating in many activities with their peers in surroundings that provide comfort and familiarity. This model enables all our residents with Alzheimer’s and other forms of memory impairment to comfortably age in our community.

      Life-Enriching Outings.

With so many things to see and do in Johns Creek and the surrounding area, we schedule outings every week with staff and other support necessary to enjoy them. Our community bus takes residents to a wide range of exciting social and cultural destinations, such as museums, the theater, restaurants and shopping areas.
